Before
Your Appointment
1. Complete the pre-visit registration link sent via text or email. This will allow you to complete the required forms and enter your medical history prior to your visit.
This is sent to your via email 6 days & 4 days prior to your appointment and via text 3 days & 1 day prior to your appointment.
2. New patients – Due to new patient appointments being in high demand, please note we require all new patients to confirm their appointment 2 days prior to avoid risk of cancellation.
All new patient paperwork will be electronic on your pre-visit registration link. If your registration is complete, please arrive 15 minutes prior to your appointment for check in. If you experience difficulty completing the registration, please arrive 30 minutes early. You will need to bring your insurance card, drivers license & method of payment. Please see our policy of accepted forms of payment.
Established patients – If your pre-visit registration is complete, please arrive 10 minutes early to your appointment or 20 minutes early if you have difficulty completing the registration.
Please notify us ahead of time if you have any new insurance changes.
3. Obstetric patients – Patients are allowed to have 1 guest accompany the patient to their appointment, including ultrasounds. Please limit additional guests due to lack of space.
4. If you have had any flu like symptoms, cough, shortness of breath, fatigue, fever or GI symptoms leading up to your appointment, please contact our office to reschedule your appointment.

To help us focus more of our time on your health and less on administrative processing, we have transitioned to a cashless and checkless practice. Financial Information
All payments are required at the time of service, this includes deductibles and copays. Please note we have transitioned to a cashless/checkless practice. Please read our full policy for details.
If you are a self-pay patient, payment is due in full at time of service to receive the discount. Please know if we verify benefits on your behalf as a courtesy, it is only an estimate provided by your insurance company and full patient responsibility cannot be determined until a claim is processed. You are responsible to know your own individual benefit coverage.
Cancellation &
No Show Policy
After 2nd offense, a $80 cancellation/no show fee will be applied for all no shows or appointments that are cancelled less than 24 hours prior to the appointment. All no show fees must be paid in order for another appointment to be made. More than 2 no shows can run the risk of the patient being dismissed from the practice.
